Festoons or malar bags are pouches of swelling that appear beneath the eyes or on the cheekbones. They are often caused by fluid retention, ageing, or simply hereditary, and typically worsen over time. This leads to more pronounced puffiness and aesthetic concerns.
Subcutaneous (internal) laser treatment of malar bag tissue using an ultra-thin optical fibre stimulates reduction of the malar bag and associated fluid.
This non-surgical approach is the most advanced technological solution for treating this difficult cosmetic issue, and without the risk of incisions or scars.

The Bayati Clinic is a specialist centre for Endolift Laser treatment of malar bags and festoons. Dr Bayati performs the largest number of subcutaneous laser malar bag cases in the UK. His pioneering technique using ultrasound guidance improves the cosmetic results and safety of the procedure.
Endolift Laser is a minimally invasive procedure used to treat malar bags and festoons by tightening the skin and shrinking the fat tissue. The procedure is performed using an ultra-thin fibreoptic laser wire, which punctures the skin without incision.
This procedure is ideal for patients seeking non-surgical options with minimal downtime. It offers noticeable results in both skin texture and malar contour. We have pioneered ultrasound guided Endolift Laser treatment for malar bags and festoons to maximise the cosmetic and safety outcomes.
Laser energy is delivered directly under the skin in the region of the malar bag or festoon. The laser stimulates inflammation and collagen production, which in turn promote closure of the fluid bag as well as improving skin firmness and texture. This results in visibly reduced appearance of the malar bag or festoon, and smoother skin under the eyes.
In some patients with malar prominence due to excess fat in the region, laser treatment can be used to partially melt the fat tissue to reduce the malar fullness.
Only one treatment is often required, with results lasting several years before repeat treatment is required.
In patients with more pronounced malar bags or festoons, we may recommend a combination of treatments that may include:
Endolift Laser
Subcutaneous (internal) laser using an ultra-thin fibreoptic wire to stimulate skin tightening and reduction of the fluid bag. We utilise ultrasound guidance to accurately target the correct tissue depth. This is performed with local anaesthesia to minimise your discomfort during treatment.
Polynucleotide Injections
Skin biostimulators encourage tissue healing and collagen production. This may be recommended to promote further collagen production and skin tightening after Endolift Laser.
Fractional CO2 Laser
CO2 laser resurfacing is occasionally recommended 3-6 weeks after Endolift Laser to stimulate further skin tightening and texture improvement over the malar bag or festoon. This is performed with topical cream anaesthesia.
At every step of your treatment, we guide you through your aftercare period and provide you with the necessary topical recovery products. Our Calecim® stem cell serum is designed to accelerate skin healing and minimise your downtime.
Follow-up appointments with Dr Bayati are arranged after your laser treatment. We review the results with you and advise any onward treatment, if recommended.
CO2, short for carbon dioxide, is the core element of this innovative treatment. The CO2 laser emits powerful pulses of light that create controlled thermal damage to the skin’s surface. Consequently, this process triggers the body’s natural healing response. This response, in turn, stimulates collagen production and skin regeneration. Ultimately, such regeneration leads to a remarkable improvement in various skin concerns, including the reduction of wrinkles, scars, and other signs of aging.
The duration of a CO2 laser treatment session typically varies based on the area being treated and the severity of skin concerns. Sessions generally last anywhere from 15 minutes to an hour.
Furthermore, the recovery period post-treatment can range from 1 to 2 weeks, depending on the specifics of your treatment. During this sensitive time, the skin is particularly vulnerable. To prevent infections, it is crucial for patients to adhere strictly to the doctor’s guidelines. Consequently, after the treatment, patients often experience a burning sensation, which can last for a few days. To alleviate this discomfort, we recommend using sterilized ice packs, which are available for purchase online or at pharmacies.
Typically, the desired results become noticeable after just one treatment. In the days following the procedure, you can expect your skin to gradually appear softer, smoother, and with diminished unwanted pigmentation. Additionally, improvements in fine lines, wrinkles, age spots, and uneven texture will start to become evident. However, to achieve the final results you’re aiming for, it’s essential to have a discussion with our doctor. This ensures a proper assessment and the creation of a correct treatment schedule tailored to your specific needs.
Ideal candidates for CO2 laser have age spots, fine lines, wrinkles, discolouration, rough texture (acne scars). Therefore, it is ideal for anyone seeking younger-looking skin with treatments that have limited downtime.
You can expect some moderate discomfort during the treatment. However, the anaesthetic cream applied before the procedure typically makes any discomfort tolerable.
You will likely experience a mild burning sensation in the treated area and your face will develop some skin swelling. This is normal and should subside after around 24-48 hours. Occasionally, some patients will notice tiny superficial bleeding points, which again is normal depending on the intensity of laser energy administered.
Over the next few days, your face will start to feel tight. Areas of bleeding previously encountered will now start to crust and heal.
It is not unusual for your face to start feeling a little itchy. However, it is crucial that you try to avoid the temptation to scratch, as this could delay healing and increase the risk of scarring. The Dermol face wash and Biafin emulsion cream that we have supplied to you help reduce the sensation of itching.
After approximately ten days, your skin will have largely healed and be free from crust. It will still remain pink for a little while longer as new skin is being born, and this will settle progressively over the following weeks.
