INJECTION ANATOMY • A millimetre or two can separate safe filler placement from vascular occlusion in some cases.
This ultrasound demonstrates the nose vascular network arching directly above the typical injection path. Nasal vascular anatomy is highly variable, which is why I do not perform blind injections in this area. This is why I only perform nose filler corrections under ultrasound guidance.
Ultrasound vascular mapping does not eliminate the vascular risk fully, but it can significantly minimise it by informing the injection strategy and confirming safe filler placement afterwards. So... in higher risk patients, filler can often still be performed like in this case but requires a well-planned image guided approach.
To me, this is as much a radiological procedure as it is a cosmetic one.

ENDOLIFT FACIAL REMODELLING • Central facial debulking, jawline definition, submental/neckline contouring and midface acne scar reduction. This is achieved from a single session of Endolift laser, results at 10 weeks. 1.5ml of filler is used to correct the sub-tear trough, smile and marionette lines.
Most of my younger Asian patients don`t have an issue with skin laxity. In these cases I use Endolift laser primarily for facial remodelling, contour enhancement and subtle slimming. Ethnic variations in tissue structure matter, and some will see more superior results of facial remodelling for this reason.

EVOLUTION OF ENDOLIFT EFFECTS • Tightening, contouring, remodelling effects of Endolift become very noticeable by 12 weeks, peak effects at around 6-9 months and then remain sustained for several years thereafter. Length of results are influenced by each individual`s biological remodelling potential, lifestyle, weight stability etc. An average patient of mine will usually see skin tightening effects beginning to diminish at around 3 years and could enquire about repeat treatment.
There is a misconception about fat reduction and energy-based treatments but this is hugely experience-based. Treating the wrong tissue planes, heavy energy use or poor clinician judgement over which areas to reduce (or preserve) fat cause undesirable volume reduction. Thoughful, creative use of Endolift and understanding which areas to preserve fat and where to reduce help prevent this (and in some patients total fat-preservation). Just like in this patient.

Endolift mid-lower face remodelling and skin tightening. Reduction in central cheek heaviness, nasal fold heaviness and lower face tightening/contouring. I now use PRF in most cases to boost the collagen remodelling response after Endolift and CO2 laser to enhance the regenerative skin improvements even more.
Preserving fat where it matters is crucial and requires experience and a creative approach to treatment. Ultrasound-guidance fine tunes areas of fat reduction by directly visualising depth of treatment and guiding which areas to deliver laser energy versus areas to avoid. PRESERVE WHAT MATTERS • Skin tightening can be exclusively achieved with preservation of fat using Endolift. This is important in patients where volume reduction may be cosmetically detrimental. This is an example of Endolift treatment for tightening only, with preservation of fat. I often perform my fat-preservation Endolift technique using ultrasound guidance so that the tissue depth of treatment is as accurate as it can be in these patients. This means undesirable loss of fat is almost non-existent.
The nasolabial folds were heavy and therefore selectively debulked to reduce the central-heavy facial appearance. Overall a super natural lift effect.
